New Student Registration

What to Expect

After the lottery in March, 2022, families will be notified if their child has been offered a seat for the 2022-2023 school year or if they have been placed on the waitlist.

If your student has received an offer, you will be receiving text and email notification with this information: At EMSA, we believe that students and teachers will reach their highest potential through active exploratory learning and social responsibility. Students will experience the WOW and WONDER of an engaging curriculum and develop our four habits of scholarship: courage, creativity, respect, and responsibility, that will mold our young learners into active contributors to a better world. We are proud to welcome your child to our school and cannot wait to meet you and your family in the upcoming months.


Step ONE:

Two Proofs of Residency will need to submitted in your online registration.

Proof 1: Please provide ONE photo from the below list

  • Most recent property tax bill and proof of payment, e.g., canceled check or Form 1098                
  • Mortgage papers
  • Signed and dated lease and proof of last month’s payment, e.g., canceled check or receipts  
  • Letter from manager and proof of last month’s payment, e.g., canceled check or receipt

Proof 2: Please provide ONE photo from the below list

  • Driver’s license
  • Vehicle registration
  • Voter registration
  • Current cable television and/or credit card bill
  • Current public aid card with the current address
  • Current homeowners/renter’s insurance policy and premium payment receipt
  • Most recent cable, gas, telephone, electric, and/or water bill
  • Matricula Card

Activity Fee: $60.00

Non-refundable Activity Fee payment of $60.00
You may pay by Cash, Check, or Credit Card

  • Pay online with PowerSchool.
  • Or in person at Elgin Math and Science Academy, 1600 Dundee Ave. Elgin, IL 60120. Make checks payable to: EMSA, with your student’s first and last name on the memo line.

By completing the above registration steps, you will officially enroll your child for the 2022-2023 school year. If you need assistance, please call: (630) 883-5013.


Step TWO: FORMS NEEDED BY AUGUST 01, 2022.

NEW STUDENTS: Birth Certificate

NEW STUDENTS must submit an ORIGINAL, certified copy of your student’s birth certificate. This birth certificate may be obtained from the County in which your child was born. If you do not have a certified copy of your student’s birth certificate, please be sure to obtain one prior to August 1. A staff member will need to see the certificate and make a copy on site to put in your student’s file.

ALL STUDENTS: Recent Physical 

All students entering EMSA in our first year are required to have a current physical examination completed on the approved Illinois Department of Public Health Form provided by your child’s physician. A current physical is one that was completed within one year of August 2020. A sports physical does not meet this requirement.


Eye Exam

The eye exam is required for all students entering Kindergarten or students enrolling for the first time in an Illinois school.

Dental Exam

Optional FREE dental screenings will be available at EMSA. The dental exam is required for all students entering Kindergarten, 2nd and 6th grade students.

Registration Fee Payment: $145.00*

Student Registration Fees (NOT the $60.00 activity fee).
We will accept Cash, Check, Money Order or Credit. If you have questions concerning your student’s registration fee or need to schedule a payment plan, please contact us by phone: (630) 883-5013.

Free or Reduced Lunch Form

The Department of Education through the National School Lunch Program offers free and reduced price breakfast and lunches to students who qualify based on family income. Online forms are available here through our school lunch system, Titan. Paper forms are available at the front desk. You may also download the documents below. The free and reduced price meal program guidelines will also be used to determine eligibility for a student fees waiver*.
